Creditor Matrix

CREDITOR MATRIX. This form is used in bankruptcy proceedings to create a creditor matrix, which is a required list of all creditors that must receive notice of the case. The form provides designated fields for entering the names and addresses of creditors in a standardized format so that the information can be properly uploaded into the court’s Case Management/Electronic Case Filing (CM/ECF) system. Each creditor’s address should be listed in no more than four lines, including the name, street address, city, state, and zip code. After completing entries for one creditor, the user continues with the next creditor in the following section. The instructions also specify that certain special characters are not permitted, as their use could prevent the matrix from uploading correctly.
