This is an Indiana appellate form used by the trial court clerk to certify that the Clerk’s Record in an appeal has been assembled and completed in accordance with Indiana Appellate Rule 10(C). The notice informs the appellate court and all parties whether the transcript has been completed, is pending, or was not requested. A certified copy of the updated Chronological Case Summary (CCS) must be attached to this notice. Once issued, this form starts the clock for the appellant’s filing of the Appellant’s Brief under Appellate Rule 45(B)(1).
