Local Form C-25 - DEFAULT JUDGMENT CHECKLIST. This form is used by the Superior Court of California, County of El Dorado to inform a party—typically the plaintiff—of the documentation required before the court will enter a default judgment in a civil case. It is issued pursuant to California Rules of Court, Rule 3.1800, and El Dorado Superior Court Local Rule 7.10.07B. The checklist outlines specific items that must be filed, including the original proof of service, a Request for Entry of Default and Judgment (Form CIV-100), a declaration substantiating the debt amount, assignment documents if the plaintiff is a collection agency, and a detailed interest calculation. Additional required documents may include a dismissal of any non-targeted parties, a case summary (excluding unlawful detainer cases), a proposed form of judgment (Form JUD-100), the original contract or promissory note, and a declaration justifying attorney fees. If any of these items are missing or incomplete, the form directs the plaintiff to resubmit the appropriate materials along with a self-addressed stamped envelope.
