AFFIDAVIT OF DOMESTIC PARTNERSHIP REGISTRATION. This form is used to register a domestic partnership in Sarasota County, Florida, pursuant to Chapter 62, Article XII of the Sarasota County Code of Ordinances. The form must be completed and submitted in person to the Sarasota County Clerk of the Circuit Court and County Comptroller for recording into the county's official records. By signing the affidavit, both individuals affirm under penalty of perjury that they meet specific legal criteria: each is at least 18 years old and competent to contract; neither is currently married or in another domestic partnership or civil union; they are not related by blood as defined by Florida law; they consider each other immediate family and are jointly responsible for each other's care and support; and they share a mutual residence in Sarasota County.
