PTO/SB/81A - PATENT - POWER OF ATTORNEY OR REVOCATION OF POWER OF ATTORNEY WITH A NEW POWER OF ATTORNEY AND CHANGE OF CORRESPONDENCE ADDRESS. This form is used by patent applicants or patent owners to manage legal representation and official correspondence for an issued U.S. patent. This form allows the user to revoke any previously granted powers of attorney and either designate new practitioners—by name or customer number—to act on their behalf before the U.S. Patent and Trademark Office (USPTO), or confirm an existing power of attorney. The form also provides the option to update or change the official correspondence address associated with the patent, either to one linked with a specific customer number or to a separately provided address. It requires the signature of the applicant or patent owner, or their authorized representative if the applicant or owner is a legal entity. In cases involving multiple patent owners or applicants, additional forms may be submitted to capture all required signatures. This collection of information is required by 37 CFR 1.31,1.32, and 1.33. The information is required to obtain or retain a benefit by the public, which is to update (and by the USPTO to process) the file of a patent or reexamination proceeding. Confidentiality is governed by 35 U.S.C. 122 and 37 CFR 1.14. This collection is estimated to take 15 minutes to complete, including gathering, preparing, and submitting the completed application form to the USPTO.
