Fee Address Indication Form {PTO-AIA-47}

PTO/AIA/47, “FEE ADDRESS” INDICATION FORM. This form is used by the United States Patent and Trademark Office (USPTO) to establish a fee address when correspondence related to maintenance fees should be mailed to a different address than the correspondence address for the application. The issue fee must have been paid for the application(s) listed on this form. In addition, only an address represented by a Customer Number can be established as the fee address for maintenance fee purposes (hereafter, fee address). If the applicant already has a customer number for the desired fee address, they should check the first box on the form. However, if the applicant does not have a customer number for the desired fee address, they must check the second box on the form and attach a completed Request for Customer Number form (PTO/SB/125) to the form.
