DWC OPEN RECORDS REQUEST FORM. This Kentucky Department of Workers’ Claims form allows individuals or organizations to request access to claim-related documents under Kentucky’s open records laws. Requestors must provide their name, company (if applicable), contact details, claimant name, claim number, and last four digits of the Social Security number. Available options include requesting the entire claim file, only the specified claim, first report of injury (requires a signed release form), agreements and awards, or certified records (additional $5 fee). All documents are redacted unless accompanied by a release form. A cost estimate/invoice is issued, and pre-payment is required before records are mailed.
