Form 22 - PETITION (CERTIFICATION AS A QUALIFIED ADOPTIVE PARENT) (PRIVATE-PLACEMENT). This is a New York Family Court form used by prospective adoptive parent(s) seeking court certification as qualified to assume custody of a child prior to, or at the same time as, filing a private-placement adoption petition. The petition includes detailed personal information such as the petitioner’s residence, marital status, physical and mental health, financial resources, and criminal history. Petitioners must disclose whether they have been involved in child protective or abuse proceedings and whether any prior certification applications have been made. A pre-placement investigation must be conducted by a disinterested person (as defined by DRL §115-d), with a written report sent directly to the court. Petitioners may request the court to appoint such an investigator. Fingerprint cards are required for a state criminal history check under DRL §115-d(3-a).
