This form is used in the Superior Court of Arizona in Coconino County by individuals who are unable to afford court-related fees in a civil, family, or probate case. It allows a party to request either a temporary postponement of court fees (a fee deferral) or a permanent cancellation of such fees (a fee waiver), depending on their financial circumstances. The form requires the applicant to identify which specific court fees they cannot afford and to provide proof of eligibility through documentation of government assistance (such as SSI, TANF, or food stamps), legal aid, or by completing a detailed financial questionnaire if documentation is not available. Applicants must list their income, household size, expenses, and assets. If service of process or publication is also unaffordable, a separate affidavit must be filed.
