Transcript Request Form {RP-003}

RP-003 - TRANSCRIPT REQUEST FORM. This form is used by individuals seeking official transcripts of court proceedings. To initiate the process, the requestor must fully complete the form with details such as the case name and number, department number, court reporter’s name, the specific dates of the proceedings, and the type of hearing or event (e.g., trial testimony, motion, plea, sentencing). If multiple court reporters were involved, a separate form must be submitted for each one. Once the form is completed, it should be emailed directly to the appropriate reporter or the reporter should be contacted via phone or through the Court Reporters Board. Reporter contact information is generally available on the Court’s website. After receiving the request, the court reporter will provide an estimate of the cost and delivery timeline, along with payment instructions. Full payment must be made before the transcript is delivered. Requests for transcripts of confidential or juvenile proceedings require judicial approval and must be submitted using Local Form RP-002.
