CTB-LF241 - DEBTOR'S NOTICE OF CONVERSION OF BANKRUPTCY CASE FROM CHAPTER 12 TO CHAPTER 7 PURSUANT TO 11 U.S.C. § 1208(a) and FED. R. BANKR. P. 1017(f)(3). This form is used in the United States Bankruptcy Court, District of Connecticut, by a debtor who voluntarily chooses to convert their ongoing Chapter 12 bankruptcy case to a Chapter 7 case. Chapter 12 is designed for family farmers or fishermen with regular annual income, and conversion to Chapter 7—liquidation—is allowed at the debtor’s discretion, without the need for a court hearing or order, as long as the case has not previously been converted. The form includes the date of the original Chapter 12 filing, the name of the trustee, and a statement confirming that the debtor is eligible for Chapter 7 relief and is acting in good faith. It also includes a certification of service showing how and to whom the notice was distributed—whether electronically, by mail, or through other means. The accompanying filing fee must be paid at the time of submission. This form is MANDATORY.
