REQUEST FORM FOR INTERNET IMAGE REMOVAL. This form is used to request the removal of specific sensitive documents from the Lake County Clerk of Circuit Court’s publicly accessible website, in accordance with Florida Statute 28.2221(5)(b). This form allows an “affected person” to request the removal of images of documents such as military discharge records, death certificates, and documents related to matters governed by the Florida Rules of Family Law, Juvenile Procedure, or Probate. The form requires identification information for each document, such as the instrument number, book and page number, or document type. Requests must be submitted in writing and may be delivered in person, by mail, fax, or email. There is no fee for this service.
