CR-240 - VICTIM NOTIFICATION CARD. This is a confidential Wisconsin form that allows victims in a criminal case to indicate whether they wish to receive future notifications regarding the defendant’s post-conviction activity. The card covers petitions for discharge from probation, release to extended supervision, or sentence adjustment under several Wisconsin statutes. Victims provide their contact information, select “yes” or “no” for notification preferences, and return the form to either the District Attorney’s Victim Witness Unit or the Clerk of Court, depending on the case. All information is kept confidential and is not accessible to defendants, attorneys, or the public.
