SOCIAL SECURITY CASE CONSENT FORM. This form is used in the United States District Court for the Western District of Oklahoma for parties involved in Social Security appeals. Pursuant to General Order 23-1, such cases are initially assigned to a United States Magistrate Judge. The form allows the parties to indicate whether they consent to have the Magistrate Judge handle all aspects of the case, including issuing final judgment and resolving any post-judgment matters. If all parties consent, any appeal will go directly to the Tenth Circuit Court of Appeals. If any party declines to consent, the case will be reassigned to a District Judge.
