REQUEST FOR ADMISSION PRO HAC VICE. This form is used in the United States District Court for the Western District of Oklahoma by attorneys who are not admitted to practice regularly before this court but wish to appear in a specific case. “Pro hac vice” admission allows an out-of-state or otherwise unadmitted attorney to participate in a particular matter with the court’s permission. The applicant must provide their full name, bar membership number, business contact information, a list of all courts or bar associations where they are currently in good standing, and disclose any history of disciplinary action or pending proceedings that could affect their professional standing. The applicant must affirm their familiarity with the Federal Rules of Evidence, Civil or Criminal Procedure, and the local rules of the court.
