CONFIDENTIAL INFORMATION FORM. This form used by the Superior Court of the District of Columbia is a required filing that accompanies an initial pleading in most civil, family, and domestic violence cases, except for charging documents. Its primary purpose is to collect sensitive and personally identifying information—such as a party’s full name, date of birth, contact details, race, ethnicity, Social Security number, and other demographic data—that is necessary for the court’s internal case management system. The form is strictly confidential and is never made part of the public record; it is returned to the filer or destroyed shortly after the information is entered into the system, as directed by Administrative Order 15-14. This form also gathers information specific to domestic violence and family court cases, such as employment details, arrest history, service instructions, interpreter or ADA accommodations, and concerns about personal or child safety. It further allows parties to indicate whether they are enrolled in the Address Confidentiality Program and to provide substitute addresses when needed. For family court filings, it also asks for children’s names, related cases, and attorney information.
