FORM 18.4 - JUDGMENT ENTRY FINDING CONSENT NOT REQUIRED. This form is used in adoption proceedings in the Probate Court of Wood County, Ohio, when the court determines that the consent of a specific individual—typically a biological parent—is not necessary for the adoption to proceed. This determination is made under Ohio Revised Code section 3107.07. The form is completed after the court finds, through testimony and evidence, that the individual in question either failed to maintain more than minimal contact with the child for at least one year prior to the adoption filing, failed to provide meaningful and consistent support as required by law or court order during that time, or meets other statutory criteria outlined in R.C. 3107.07.
