FORM 19.10 - APPLICATION BY RELATIVE FOR APPROVAL OF PLACEMENT. This form is used in the Probate Court of Wood County, Ohio, when a relative of a child seeks court approval to place the child for adoption under Ohio Revised Code section 5103.16(D). This form allows a relative—such as a grandparent, aunt, uncle, or other family member—to formally request the court’s authorization to place the child, asserting that the placement would be in the best interest of the child. The applicant must provide their name, address, relationship to the child, and details about the child, including their date of birth and current residence. The form also requires the applicant to indicate if one or both of the child’s parents are deceased or have abandoned the child, as defined under Ohio law. The applicant agrees to undergo an independent home study to evaluate the suitability of the placement and requests that the court schedule a hearing to consider the proposed placement.
