CANCELLATION OF MARK (PURSUANT TO NRS 600.390). This form is used to officially cancel a registered mark with the Nevada Secretary of State. This form applies to trademarks, trade names, or service marks that were previously registered in Nevada. Reasons for cancellation may include voluntary withdrawal by the registrant or assignee, abandonment of the mark, incorrect ownership, improper or fraudulent registration, or a likelihood of confusion with another registered mark. A court order may also be grounds for cancellation. The form must include the name of the registered mark, its registration number, type, original registration date, and the specific reason for cancellation.
