AMENDMENT OR CORRECTION OF MARK (PURSUANT TO NRS 600.355). This form is used to correct or amend an existing trademark, trade name, or service mark registration with the Nevada Secretary of State. This form allows a registrant or their legal successor to identify and correct any inaccuracies in a previously filed registration. It requires the name of the registered mark, the type of registration, the original filing date, a description of the error, and the corrected or amended information. The form must be signed by the registrant or their successor and submitted with a $60.00 filing fee.
