APPLICATION TO PROVIDE SERVICES TO THE HEARING IMPAIRED OR OTHER PERSONS WITH COMMUNICATION DISABILITIES. This is a court accessibility form used in the U.S. Bankruptcy Court for the Eastern District of New York (EDNY). This form allows parties—including debtors, plaintiffs, defendants, and witnesses—to request court-provided sign language interpreters or auxiliary communication aids for hearings, trials, or other proceedings.
