This form is used in the Eighth Judicial Circuit, Alachua County, Florida, for guardians or guardian advocates to provide a yearly report on the well-being of a ward with a developmental disability. It is a required submission to the court that details the ward’s current living situation, medical care, social activities, and overall condition.
