ANNUAL GUARDIANSHIP PLAN FOR MINOR. This form is used in the Eighth Judicial Circuit, Alachua County, Florida, for guardians of minors under court-ordered guardianship. It serves as a yearly report submitted to the court detailing the minor ward’s well-being, living arrangements, medical care, education, and social development. The form requires the guardian to provide the ward’s current address, as well as a history of residences over the past year, including the names and locations of any facilities or homes where the minor has stayed. It also documents any medical or dental treatments the ward has received, along with a physician’s report evaluating the ward’s physical and mental condition, which must have been conducted within 180 days before the reporting period. The form outlines plans for the ward’s future medical and dental care, summarizes their educational progress, and describes their social development, including their ability to communicate and maintain relationships. If the ward is 14 years of age or older, the guardian must confirm that they have consulted with the ward regarding the plan and, to the extent possible, considered their wishes.
