KC - CERTIFICATE OF CANCELLATION DOMESTIC (KANSAS) LIMITED LIABILITY COMPANY. This form is used to formally terminate the existence of a limited liability company (LLC) that was formed and registered in the state of Kansas. This form must be submitted when an LLC has decided to cease operations and cancel its legal status with the Kansas Secretary of State. To file this form, the LLC must be in good standing or listed as “registered” with the Secretary of State; if the company is in forfeited status, it must first complete a reinstatement before cancellation. The form requires the business’s identification number, the legal name of the LLC as registered with the state, and a statement of the reason for cancellation. It must be signed by an authorized representative of the LLC. If the company has created one or more designated series under its formation, an additional attachment listing each series along with its ID number is required, and a fee of $20 per series must be paid.
