Description
DC 6:14.4 - COMPLAINT FOR MODIFICATION OF CHILD SUPPORT (DECREASE). This form is used in the Nebraska District Court by a party seeking to reduce an existing child support order. The person completing the form, who may be the original plaintiff or defendant, provides information about the original child support order, including the date it was issued, the name of the person ordered to pay, the children covered under the order, and the monthly support amounts. The form is used to inform the court that there has been a material change in circumstances—such as a significant reduction in income, unemployment, disability, or reliance on Supplemental Security Income (SSI)—that justifies a decrease in child support payments. The change must have lasted for at least three months and is expected to continue for at least six more. The person requesting the modification must also show that applying the Nebraska Child Support Guidelines to the current financial situation would result in a decrease of at least 10% and not less than $25 per month. The form must be signed, dated, and include contact information, and it must comply with Nebraska Supreme Court rules requiring self-represented parties to provide an email address unless they are unable to receive emails. Neb. Rev. Stat. § 42-364, Neb. Ct. R. § 4-217. www.FormsWorkflow.com
