TAX DISCLOSURE REPORT - DOMESTIC PROPERTY AND CASUALTY INSURANCE COMPANIES. This form is used by domestic property and casualty insurance companies to report various financial details related to their business operations in Massachusetts. It requires the company to provide information about its principal office location, gross receipts or sales, premiums and income taxable in Massachusetts, and the excise or tax due. Additionally, the company must disclose any tax credits taken against its excise obligations under Massachusetts General Laws, Chapter 63.
