040 - REQUEST FOR MARYLAND RELEASE OF FOREIGN BEER. This form is used by U.S. importers to request authorization from the Maryland Comptroller’s Office to release shipments of foreign beer into the state of Maryland. This form is required whether the beer is bottled, canned, or in kegs. It collects detailed information about both the U.S. importer and the authorized Maryland importer, including license numbers and addresses. The form requires the importer to list each brand of beer being released, the number of cases or kegs, container sizes, gallons per unit, and to calculate the total gallons and the tax due at the rate of $0.09 per gallon. The tax must be remitted either online through MarylandTaxConnect or by check or money order payable to the Comptroller of Maryland.
