ORDER GRANTING APPLICATION FOR PAYMENT OF UNCLAIMED FUNDS. This form is used by the United States Bankruptcy Court for the Northern District of Ohio to authorize the release of unclaimed funds that were previously deposited with the court. When a party—referred to as the Claimant—files an application requesting the release of these funds under 11 U.S.C. § 347(a), the court reviews the application and supporting documentation to determine if the claimant is legally entitled to receive the funds. If the court finds the request valid, it issues this order directing the Clerk of Court to release the specified amount to the claimant at the designated address.
