PETITION FOR GUARDIAN. This form is used in the Circuit Court of the 20th Judicial Circuit, St. Clair County, Illinois, in probate cases involving a minor. The petitioner, a reputable citizen of Illinois, submits the petition on oath and provides the following information: Details of each minor, including name, date of birth, residence address, and county. Approximate value of the minor's personal estate and anticipated annual income. Information about the person currently having custody of the minor.
