Foreign Name Registration Application {1-R}

FORM 1R - FOREIGN NAME REGISTRATION APPLICATION (ANNUAL). This form is used by foreign business entities—those formed outside the State of Wisconsin—that wish to reserve the right to use their business name within Wisconsin without yet obtaining a certificate of authority to transact business in the state. This form, filed with the Wisconsin Department of Financial Institutions, allows entities to either apply for an original name registration or renew an existing name registration for the upcoming calendar year. The form requires the legal name of the foreign entity, the jurisdiction and date of its formation, and, if applicable, a fictitious name that complies with Wisconsin’s entity naming requirements when the entity’s true name is not available or distinguishable in state records. Name registrations are valid through December 31 of the year filed and must be renewed between October 1 and December 31 to avoid expiration. The application must be signed by an authorized individual on behalf of the entity and is submitted with a $50 filing fee. Optional expedited processing is available for an additional $25.
