ADR-003 - NOTICE OF MEDIATOR ASSIGNMENT. This form is used in Inyo County Superior Court (California) notifying a mediator of their appointment to a specific case under the court’s Alternative Dispute Resolution (ADR) program. The notice identifies the case, assigns the mediator, and instructs them to contact the ADR Administrator within seven (7) calendar days if they are unable to serve due to a conflict of interest or other valid reason. The form also establishes a deadline for completion of the mediation session and requires the mediator to file the Statement of Agreement or Nonagreement (Form ADR-100) with the ADR Administrator within ten (10) days after the mediation concludes.
