APPEARANCE AND WAIVER. This form is used in Mono County Superior Court and allows an attorney to appear on behalf of a defendant without requiring the defendant’s personal appearance at arraignment. The form includes a waiver of formal arraignment, entry of not guilty pleas, and denial of any prior convictions or probation violations.
