CERTIFICATE OF CORRECTION. This is a California Superior Court (Mono County) form used to show proof that correctable vehicle-related violations—such as equipment, registration, or driver’s license issues—have been fixed. Typically used when a “fix-it” ticket is lost, the form allows a law enforcement officer or authorized inspection agent to certify that the violation has been corrected. Once completed, the form must be submitted to the court along with a required transaction fee for each violation. Upon receipt and verification, the court may dismiss the violation.
