FL-5 - STIPULATION FOR SUBSEQUENT MEDIATION. This is a Napa County Superior Court family law form used when both parties agree to participate in additional mediation through Family Court Services after a prior mediation referral. The form confirms that the request is being made within one year of the last mediation referral, that participation is voluntary, and that both parties understand a new Request for Order must be filed if mediation does not resolve the dispute.
