ABSENTEE REPORT FORM. This form is used by employees to report their absence from work. It records details such as the employee's name, employee number, department, shift, the dates and times of absence, whether the absence was with or without permission, and the reason for the absence (e.g., accident, illness, vacation, personal affairs, jury service, no work, or other).
