JD-FM-178 - AFFIDAVIT CONCERNING MILITARY SERVICE. This is a Connecticut Judicial Branch Superior Court form used to provide information about a defendant’s military status when the defendant has not filed an Appearance with the court. The affidavit may be completed by anyone who has knowledge of the defendant’s military status and must be signed under oath before a court clerk, notary public, or attorney. The form allows the affiant to certify that the defendant is in the U.S. military, is not in the military and explain the basis for that knowledge, or that the military status is unknown.
