BMV 4853 - HISTORICAL “MODEL YEAR” LICENSE PLATE AFFIDAVIT. This form is used by vehicle owners who wish to display an original Ohio license plate from the model year their historical vehicle was manufactured. Under Ohio Revised Code §4503.181, individuals who have registered their vehicles with historical license plates may apply to display a single, authentic, and legible model-year plate corresponding to the vehicle’s year of manufacture. The plate must be an original issued by the state of Ohio, and its design must match the original color scheme for that year. While a model-year plate may be displayed on the rear of the vehicle, the vehicle must still be registered with distinctive historical plates, and the registration card must be kept in the vehicle and available for inspection by law enforcement. This form requires applicants to provide documentation such as a copy of the model-year plate, historical registration, and proof of insurance, as well as complete a notarized affidavit affirming the plate's authenticity and their compliance with financial responsibility laws.
