This form is issued by the Delaware Superior Court to approve a proposed tort settlement on behalf of a minor when the settlement funds are to be placed in a structured annuity. This order authorizes the petitioner to accept the settlement amount, execute a general release, and distribute approved legal fees, expenses, and medical payments. The remaining net proceeds must be used to fund an annuity or other structured instrument for the minor’s benefit, with payments beginning no earlier than the age of majority.
