SC-26 - REQUEST AND ORDER. This is a district court form that allows either a plaintiff or defendant to submit a written request for court action in an existing case. The filer may request relief such as scheduling changes, procedural orders, or other case-related directives, and must briefly explain the reason for the request. The form includes signature and contact information fields, service requirements, and a Certificate of Service for use when TrueFiling is not available or not required.
