This form is used in the Superior Court of Arizona (Maricopa County) to finalize a divorce between spouses who have no minor children together. It records the court’s findings—jurisdiction, the 90-day Arizona residency requirement, that the marriage is non-covenant, that conciliation requirements (if any) are met, and that the marriage is irretrievably broken—and then enters binding orders that restore the parties to single status.
