Description

FINGERPRINT INFORMATION FOR GUARDIANSHIP PETITIONERS. This form provides instructions and requirements for guardianship petitioners in Alachua County, Florida to complete the mandatory fingerprinting process for a Level 2 background screening, as required by Florida Statute 744.3135(1). All individuals seeking appointment as a guardian must submit electronic fingerprints, with the initial cost borne by the petitioner. The process involves visiting the Alachua County Sheriff’s Office, presenting a valid photo ID (preferably a Florida driver’s license), and paying a $10 fingerprinting fee. The Sheriff’s Office uses a specific Originating Agency Identification (ORI) Number to ensure that the fingerprint results are sent directly to the Alachua County Clerk of Court. After fingerprinting, the petitioner must use the provided Transaction Control Number (TCN) to make a separate online payment of $37.25 to the Florida Department of Law Enforcement (FDLE). The payment must be made via the FDLE CAPS system, and confirmation of the payment, along with the completed form, must be submitted to the Clerk’s Office within 48 hours of filing the guardianship petition. www.FormsWorkflow.com
