CTB-LF241 - ERRATA SHEET REGARDING OFFICIAL TRANSCRIPT (BANKRUPTCY CASE). This form is used in the United States Bankruptcy Court for the District of Connecticut to request corrections, additions, or deletions to an official court transcript. After a hearing, trial, or evidentiary proceeding has taken place and the transcript has been filed, any party reviewing the transcript can use this form to identify specific errors by noting the page and line number, the proposed correction or change, and the reason for the request. Once the corrections are documented, the form must be signed and dated by the person requesting the changes.
