FA-4184V - DECLARATION OF EFFORTS TO SERVE RESPONDENT (SERVICE BY PUBLICATION). This form is used in the Wisconsin Circuit Court when a petitioner in a marriage or paternity case has been unable to locate and personally serve the respondent with legal documents. The form allows the petitioner to formally state, under penalty of false swearing, that they have exercised reasonable diligence in attempting to find the respondent. This includes attempts such as personal visits to the last known address, contacting relatives, or using a process server or sheriff, all of which failed to produce a valid service. If no current address is known, or if mail sent to the last known address has been returned, the petitioner may use this form to support a request for service by publication—a legal process of notifying the respondent by publishing notice in a qualifying newspaper.
