REQUEST FORM FOR SOCIAL SECURITY OR ACCOUNT NUMBER REMOVAL.This form is used to request the removal or redaction of social security numbers or account numbers—including full bank account, debit, charge, or credit card numbers—from documents that are publicly available on the Clerk’s Internet website, in accordance with Florida Statute 119.0714(3). The person submitting the request must either be the individual whose information appears in the record or be acting in a legal capacity on their behalf, such as an attorney or legal guardian. The form requires the identification of the specific document and page number where the sensitive information appears.
