This form is used in the United States Bankruptcy Court for the Western District of Texas to affirm the accuracy and truthfulness of documents submitted electronically in a bankruptcy case. This form must be signed and filed by the individual debtor, or by an authorized representative if the debtor is a business entity, at the time of electronically submitting any original or amended bankruptcy petitions, schedules, statements, or master mailing lists (also called the matrix). By signing the declaration under penalty of perjury, the filer confirms that they have reviewed the documents in question and that the information is true and correct. The declaration also serves as a procedural safeguard: failure to file it may result in dismissal of the case or rejection of the amended documents.
