Description

6:14.12 - ORDER FOR MODIFICATION (CHILD SUPPORT). This form is used in the Nebraska District Court to document the court’s decision after a hearing on a request to modify an existing child support order. It is completed by the judge once the court has reviewed evidence, heard testimony, and determined whether there has been a material change in circumstances—such as a significant change in income—that meets the legal threshold for adjusting child support under the Nebraska Child Support Guidelines. The form records important findings, including the original child support order details, the number of children affected, the amounts previously ordered, and any changes in the parents’ ability to provide health insurance or cash medical support. If modification is granted, the form specifies the new child support amount, payment schedule, and method of payment through the Nebraska Child Support Payment Center. It also outlines medical support requirements, the duration of the order, and which parts of the prior order remain in effect. This document becomes the binding court order governing child support until the child(ren) reach age 19, are emancipated, or the court issues a further modification. Neb. Rev. Stat. § 42-364, Neb. Ct. R. § 4-217. www.FormsWorkflow.com
