DC 6:14.5 - COMPLAINT FOR MODIFICATION OF CHILD SUPPORT (INCREASE). This form is used in the Nebraska District Court by a party requesting that the court increase an existing child support order. The person completing the form—who may be representing themselves without an attorney—provides details about the most recent child support order, including the date it was issued, the name of the person currently paying child support, and the children covered under the order. The form is used when there has been a material change in circumstances since the order was entered, such as a significant increase in the paying party’s income or other financial changes that justify an increase in the monthly child support amount.
