DC 6:15.3 - COMPLAINT FOR MODIFICATION OF CUSTODY OR PARENTING PLAN. This form is used in the District Court of Nebraska by a party who wishes to change the terms of an existing custody order or parenting plan. The person completing the form—who may be representing themselves—requests the court to issue a new order that reflects changes in the needs or circumstances of the child(ren) or the parents since the last court order was entered. This form is typically used when a material change in circumstances has occurred, such as a change in a parent's living situation, work schedule, health, or the needs of the child. The form requires detailed information, including the names and birth years of the children involved, the current and past living arrangements, and the legal history related to the custody or parenting time of the children. It also asks whether the parties agree or disagree on custody, parenting time, or child support. The petitioner must explain why the current arrangement no longer serves the best interests of the child(ren) and what modifications are being requested—such as a change in legal or physical custody, a revised parenting schedule, or updated child support orders.
