DC 6:15.8 - ORDER FOR MODIFICATION (PARENTING PLAN). This form is used in the Nebraska District Court when a parent requests changes to an existing custody arrangement or parenting plan. It is completed by the judge after a hearing in which the court reviews testimony, evidence, and the circumstances surrounding the child(ren)’s welfare. The form documents the court’s findings that a material change in circumstances has occurred since the original decree or prior order, and that modifying the parenting plan is in the best interests of the child(ren). It specifies the type of custody awarded—sole or joint legal and/or physical custody—and incorporates an updated parenting plan that complies with Nebraska’s Parenting Act.
