JD-FM-164 - AFFIDAVIT CONCERNING CHILDREN. This form used in Connecticut family and probate courts for cases involving child custody or visitation. This form requires the applicant to provide detailed information about each child affected by the case, including their residence history for the past five years and the individuals they have lived with. It also asks whether the applicant has been involved in any other custody or visitation cases in Connecticut or another state and requires disclosure of any legal matters, such as family violence, protective orders, guardianship, child neglect or abuse, termination of parental rights, or adoption cases, that could impact the current proceedings.
